• Digitised NPC Attestation Certificate for those born before 1992.
• Digitised NPC Birth Certificate for those born after 1992.

Documents Required for Change of Name Modification:

• Court affidavit.
• Newspaper extract.
• Marriage certificate in the case of marital status.
• Other government ID.

Documents Required for Change of Address Modification:

• Utility bill.
• Tenancy agreement.
• Community leader’s attestation letter.

Documents Required for Phone Number Modification:

• Police report (provide your phone number without “0” after the “+234” country code, for example +234 812345…).
